URGENT - Signing on for 08-09
To register to play for the U17s next season, you need to complete a
Larkhall membership application form (click link to download)
AND a
Mid Wilts player registration form (click link to download)
Please return both ASAP to Steve Horwood at 6 Avon Park, Bath BA1 3JP and no later than Sunday, 26 July.
Please note that with the Mid Wilts form you must attach a passport sized, colour photo in the top right corner AND include a photocopy of the whole form (can be B&W).
Please also be sure to include a £30 cheque for your Larkhall membership deposit.
